- A $50 fee is collected upon booking a non-surgical treatment, whether online or over the phone, that will be applied toward your treatment or products at the time of service. If you cancel your appointment within 48 hours of the appointment or no-show, the $50 is held and applied towards a late cancellation or no show fee.

Surgical Scheduling
One of our Patient Care Coordinators can help schedule your surgical consultation. There are three options for getting in contact with a PCC.
- Call us at (801) 776-2220.
- Fill out a contact form HERE and a PCC will get back to you as soon as possible.
- Schedule a phone call with a PCC at your preferred location below.
- A $150 fee will be collected by your PCC to schedule a surgical consultation. This amount can be applied toward surgery or med spa services within one year of your consultation. If you cancel your appointment within 48 hours of the appointment or no-show, the $150 is held and applied towards a late cancellation or no show fee.
